This site provides access to information/forms utilized in the school health program throughout all CASD buildings. If your child has a special health concern, please contact the nurse at his/her school anytime during the school year.
School Health Services mandated by PA Department of Health will be provided for your child on a yearly basis according to the following schedule:
Height and Weight Measurements-Grades K-12
Vision Screenings- Grades K-12
Hearing Screenings- Grades K-3, 7 and 11
Physical Exams by School Physician- Grades K, 6 and 11
Dental Exams by School Dentist- Grades K, 3 and 7
Scoliosis Exams- Grades 6 and 7
