Diabetes Management

The Connellsville Area School District is committed to providing a safe, supportive, and inclusive learning environment for students with diabetes. Students with a diagnosed diabetes condition are supported through individualized planning and evidence-based health practices to ensure access to education while prioritizing student safety.

Each student with diabetes must have a current Diabetes Medical Management Plan (DMMP) and physician’s orders on file with the school nurse. These medical documents guide school staff in recognizing symptoms of hypo- and hyperglycemia, responding appropriately, and providing any prescribed medications, insulin, or treatments.

The District’s certified school nurses oversee diabetes management in collaboration with families, healthcare providers, and school staff. Nurses provide training and guidance to designated staff members as appropriate and ensure that health plans are implemented consistently across school settings.

Diabetes management is addressed through the student’s Individualized Health Plan (IHP) and, when applicable, integrated into a Section 504 Plan or Individualized Education Program (IEP) to support access to instruction and participation in school activities.

The District complies with applicable federal and state laws and regulations related to the care of students with diabetes, including requirements for safe medication administration, staff training, and emergency response planning.

The District follows established medical standards and guidance, including recommendations from organizations such as the American Diabetes Association, to promote best practices in diabetes care and school-based management.

Families are encouraged to communicate any changes in their child’s medical condition, treatment regimen, or supplies promptly so plans can be updated.
